Campus Access Authorization Form

Due to the impact of COVID-19, all guests visiting Stonehill College must complete this form ON THE DATE OF THEIR VISIT before arriving to campus.

Upon completion of this form, you will be provided with a credential via email that will grant you access to campus. All guests must present this credential to our gatehouse attendant upon arrival to campus.

Special Exceptions

Please note the following special exceptions:

Overnight guests should not use this form. Overnight guests must be registered by current students before being provided with a link to a special-use campus access authorization form. Please ask your student host to complete the Overnight Guest Registration form in myHill prior to your visit.

Library and Archives access requires advance appointment. The MacPháidín Library and Stonehill College Archives are open to the public by appointment only. Community members who wish to visit the library should email librarydeskgroup@stonehill.edu to make an appointment. 24 hours notice is required to visit the library. Community members who wish to visit the Archives should email barchives@stonehill.edu to make an appointment. Two weeks notice is required to visit the archives.

Prospective Students visiting the Admission Welcome Center or Sally Blair Ames Sports Complex for a recruitment event or campus tour should not use this form, and should instead refer to their visit registration for specific campus access instructions.

COVID-19 Symptom Self-Checker

Within the past 5 days, I have not been placed on quarantine for possible exposure to COVID-19
Within the past 5 days, a public health or medical professional has not told me to self-isolate, or quarantine because of concerns about having COVID-19
Within the past 5 days, I have not been in close contact with someone who has been diagnosed with COVID-19 (Note: The CDC defines close contact as someone who was less than 6 feet away from an infected person for a cumulative total of 15 minutes or more over a 24-hour period.)

Waiver of Liability

This waiver and acknowledgement statement responds to the impact of COVID-19 on guests visiting Stonehill College. Wherever the term COVID-19 is used, we are referring to the disease identified as severe acute respiratory syndrome coronavirus 2 and the health conditions caused through contraction of the coronavirus.

You understand that COVID-19 is contagious and believed to spread mainly from person-to-person contact. While Stonehill College has put in place reasonable preventative measures to reduce the spread of COVID-19 throughout its campus, the College cannot guarantee that by using campus facilities, and attending this visit, or taking a tour, you will not become infected with COVID-19.

In signing this waiver, you understand the risks involved, which may include severe injury and even death. You are asked to think about your own physical health condition and personal risk when making the decision to visit campus. You agree not to hold Stonehill legally responsible if your are exposed to or contract COVID-19 unless it is the result of the College’s willful misconduct or gross negligence.
