Campus Access Authorization Form

All guests will be required to complete our Campus Access Authorization Form, which serves as a COVID-19 self-checker and waiver of liability. The form must be completed on the date of the guest's arrival to campus. Successful completion of the Campus Access Authorization Form will produce a credential via email that each visitor much present at the gatehouse in order to gain access to campus.
